Welcome to OpenExchange - OpenXDS

  This is a community project and, as such is not supported with standard SLAs. 

Please post your questions or problems via discussion forum and the community will provide their best efforts to assist. For information on the Enterprise Edition, please send an email to moss@misys.com.

 

 

 

 

About OpenExchange

OpenExchange platform provides standards based core infrastructure to exchange patient health information in a secure and timely manner, to advance the quality, safety and efficiency of healthcare delivery. The platform is a critical element of HIE infrastructure that provides clinicians and other members of the healthcare community with the right information at the right time by making patient information available across organizational boundaries. It facilitates and streamlines the sharing of patient information throughout an HIE community. 

OpenExchange consists of open source projects that use Integrating the Healthcare Enterprise (IHE) profiles as the set of fundamental requirements. IHE (Integrating the Healthcare Enterprise) is an initiative by healthcare professionals and industry to improve the way computer systems in healthcare share information. IHE promotes the coordinated use of established standards such as DICOM and HL7 to address specific clinical needs in support of optimal patient care. Systems developed in accordance with IHE standards are better able to communicate with one another and empower care providers to use information more effectively. The core set of sub-projects under OpenExchange include

  • OpenPIXPDQ - which implements the Patient Identifier Cross-reference (PIX) Manager and Patient Demographics Query (PDQ) Supplier actors.
  • OpenXDS -  which implements the Cross-Document Sharing (XDS) Registry and Repository actors, as well as the Cross Community Access (XCA) Responding Gateway and Initiating Gateway actors.
  • OpenATNA - which implements an Audit Record Repository of the IHE Audit Trail Node Authentication (ATNA) profile.
  • OpenDSUB - which implements the IHE Document Metadata Subscription profile.

OpenExchange and its sub-projects are the Open Health Tools Charter projects monitored by the Open Health Tools Board.

Licensing

OpenExchange and all its sub-projects are released under the Apache Version 2 LicenseThird-party libraries are distributed under the terms and conditions of their respective licenses.

 

About OpenXDS

The IHE Cross Enterprise Document Sharing (XDS.b) integration profile facilitates the registration, distribution and access across health enterprises of patient electronic health records. XDS.b is focused on providing a standards-based specification for managing the sharing of medical summary documents between any healthcare enterprise, ranging from a private physician office to a clinic to an acute care in-patient facility.

The IHE XDS.b integration profile assumes that these enterprises belong to one or more clinical affinity domains. A clinical affinity domain is a group of healthcare enterprises that have agreed to work together using a common set of policies and share a common infrastructure. The server side of XDS.b implementation consists of two parts - A Document repository and a registry.

  • A Document Repository is responsible for storing documents in a transparent, secure, reliable and persistent manner and responding to document retrieval requests.
  • A Document Registry is responsible for storing information about these documents so that the documents of interest for the care of a patient may be easily found, selected and retrieved irrespective of the repository where they are actually stored.

As XDS.b is document content agnostic, any type of patient clinical information without regard to content and representation is supported. This makes the XDS IHE integration profile equally able to handle documents, containing multiple types of formats including simple text, formatted text (e.g. HL7 CDA), images (e.g. DICOM) or structured and vocabulary coded clinical information (e.g. CDA Release 2, CCD) etc. In order to ensure the necessary interoperability between the document sources and the document consumers, the individual organizations must adopt policies concerning document format, structure and content.

This project mainly implements the Document Repository and Document Registry actors of the XDS.b profile mentioned above. In addition, it implements the initiating gateway and responding gateway actors of the Cross Community Access (XCA) profile.

OpenXDS leverages the NIST iheos project and the omar 3.1 project from freebxml. It has been mainly developed by Misys Open Source Solutions (MOSS). Other contributors include Cardiff UniversityIBM,  Sysnet InternationalTopicus and a number of individuals such as Abbas Raza and Syed Haider.

OpenXDS 1.0.1 Available
The binary OpenXDS 1.0.1 build is made available. Go to File Releases to download.

The changes in OpenXDS 1.0.1 from 1.0 include:
- Additional MySQL and SQL Server databases support
- Added audit log with TLS transaction
- Generic actor configuration refactoring
- Integrated with OpenExchange
- Added property and spring configuration framework
- Added configuration pre and post processes interface
- Provided OpenXDS extension hook
- Some bug fixes

OpenXDS Successfully Tested at Connectathon NA 2010.
At the 2010 Connectathon North America in Chicago last week, OpenXDS was tested and passed these IHE profiles and actors:


XDS: Document Registry, Document Repository


XCA: Initiating Gateway, Responding Gateway


XUA: X Service User, X Service Provider


ATNA: Audit Log Source


CT: Consistent Time Client


In the meanwhile, all the corresponding HITSP profiles were also passed.


There were 100 over vendors and 152 systems participated in this weeklong event. OpenXDS was successfully tested with the following 41 companies:
Agfa, Alert, Allscripts, Axolotl, Axsys, Carefx, CE, Core Point Health, CPN, CPSI, DBMotion, ECLP, EMDS, EPIC, GE, Healthvision, HMS, HSI, IBM, ICA, Infinitt, Insite1, Intel, Intersystems, Karos, McKesson, Medquist, Microsoft, MIE, Nextgen, NMC, OHT, Oracle, Patient Keeper, Philips, Quadramed, Sage, Siemens, SIS, Spirit, Vangent.
